Lumen Wellness Inc. ("Lumen", "we", "us") respects your privacy. This policy explains what personal information we collect, why, and how we protect it.
What we collect
- Contact details: your name, email, phone number and session address.
- Booking details: the date, time and type of session you book.
- Your date of birth and emergency contact information.
- High-level health information you choose to share, such as injuries or pregnancy, so we can keep your sessions safe.
- Your waiver signature and the consents you give, including whether we may use photos of you.
Why we collect it
- To book, plan and deliver safe, personalized sessions.
- To contact you about your bookings.
- To keep a record of your signed waiver and consents.
- To send news and birthday offers, only if you opt in. You can unsubscribe at any time.
Who can see it
Only your instructor at Lumen Wellness. We never sell or rent your information. We use trusted services to run bookings and forms (Google Calendar, Google Workspace and Jotform), which store information on our behalf.
How long we keep it
We keep your information while you're a client and for as long as needed for legal and insurance records, including signed waivers. After that, we delete it securely.
Your choices
You can ask to see, correct or delete your information, or withdraw a consent such as photo use, at any time by emailing hello@lumenwellness.ca. Some records, like signed waivers, may need to be kept for legal or insurance reasons.
